f69953837c scsi: qedi: Fix error return code of qedi_alloc_global_queues()
When kzalloc() returns NULL to qedi->global_queues[i], no error return code
of qedi_alloc_global_queues() is assigned.  To fix this bug, status is
assigned with -ENOMEM in this case.

96a766a789 scsi: qedi: Add support for handling PCIe errors
The error recovery is handled by management firmware (MFW) with the help of
qed/qedi drivers. Upon detecting errors, driver informs MFW about this
event which in turn starts a recovery process. MFW sends ERROR_RECOVERY
notification to the driver which performs the required cleanup/recovery
from the driver side.

4118879be3 scsi: qedi: Mark all connections for recovery on link down event
For short time cable pulls, the in-flight I/O to the firmware is never
cleaned up, resulting in the behaviour of stale I/O completion causing
list_del corruption and soft lockup of the system.

On link down event, mark all the connections for recovery, causing cleanup
of all the in-flight I/O immediately.

28b35d17f9 scsi: qedi: Fix list_del corruption while removing active I/O
While aborting the I/O, the firmware cleanup task timed out and driver
deleted the I/O from active command list. Some time later the firmware
sent the cleanup task response and driver again deleted the I/O from
active command list causing firmware to send completion for non-existent
I/O and list_del corruption of active command list.

Add fix to check if I/O is present before deleting it from the active
command list to ensure firmware sends valid I/O completion and protect
against list_del corruption.

5c35e46465 scsi: qedi: Skip firmware connection termination for PCI shutdown handler
In boot from SAN scenario when qedi PCI shutdown handler is called with
active iSCSI sessions, sometimes target takes too long time to respond to
firmware connection termination request.  Instead skip sending termination
ramrod and progress with unload path.

a7f7f6248d treewide: replace '---help---' in Kconfig files with 'help'
Since commit 84af7a6194 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.

This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.

There are a variety of indentation styles found.

  a) 4 spaces + '---help---'
  b) 7 spaces + '---help---'
  c) 8 spaces + '---help---'
  d) 1 space + 1 tab + '---help---'
  e) 1 tab + '---help---'    (correct indentation)
  f) 1 tab + 1 space + '---help---'
  g) 1 tab + 2 spaces + '---help---'

In order to convert all of them to 1 tab + 'help', I ran the
following commend:

  $ find . -name 'Kconfig*' | xargs sed -i 's/^[[:space:]]*---help---/\thelp/'

4a4c0cfb4b scsi: qedi: Check for buffer overflow in qedi_set_path()
Smatch complains that the "path_data->handle" variable is user controlled.
It comes from iscsi_set_path() so that seems possible.  It's harmless to
add a limit check.

The qedi->ep_tbl[] array has qedi->max_active_conns elements (which is
always ISCSI_MAX_SESS_PER_HBA (4096) elements).  The array is allocated in
the qedi_cm_alloc_mem() function.

For an unreachable target, offload_work is not initialized and the endpoint

2f1ea39870 scsi: qedi: Remove additional char from boot target iqnname
While parsing the iSCSI TLV data to MFW request, a newline was added to the
firmware boot target iqnname string. Because of this, we were getting the
following error even after the boot target was successfully logged in:

"[qedi_get_protocol_tlv_data:1197]:1: Boot target not set"

Remove the trailing newline.

The new route handling in ip_mc_finish_output() from 'net' overlapped
with the new support for returning congestion notifications from BPF
programs.

In order to handle this I had to take the dev_loopback_xmit() calls
out of the switch statement.

The aquantia driver conflicts were simple overlapping changes.

The phylink conflict was between a bug fix by Russell King
to make sure we have a consistent PHY interface mode, and
a change in net-next to pull some code in phylink_resolve()
into the helper functions phylink_mac_link_{up,down}()

On the dp83867 side it's mostly overlapping changes, with
the 'net' side removing a condition that was supposed to
trigger for RGMII but because of how it was coded never
actually could trigger.

Currently the qedi_dbg_* family of functions can overrun the end of the
source string if it is less than the destination buffer length because of
the use of a fixed sized memcpy. Remove the memset/memcpy calls to nfunc
and just use func instead as it is always a null terminated string.

3252688f09 scsi: qedi: Adjust termination and offload ramrod timers
Whenever offload ramrod is issued, firmware wants driver to wait for max 5
secs, otherwise driver can initiate further corrective action. Similarly,
when termination ramrod is issued, irrespective of abortive or non-abortive
termination, driver should wait for 60 sec * max TCP-RT timeout.

fb24ea52f7 drivers: Remove explicit invocations of mmiowb()
mmiowb() is now implied by spin_unlock() on architectures that require
it, so there is no reason to call it from driver code. This patch was
generated using coccinelle:

NOTE: mmiowb() has only ever guaranteed ordering in conjunction with
spin_unlock(). However, pairing each mmiowb() removal in this patch with
the corresponding call to spin_unlock() is not at all trivial, so there
is a small chance that this change may regress any drivers incorrectly
relying on mmiowb() to order MMIO writes between CPUs using lock-free
synchronisation. If you've ended up bisecting to this commit, you can
reintroduce the mmiowb() calls using wmb() instead, which should restore
the old behaviour on all architectures other than some esoteric ia64
systems.

34a2ce8876 scsi: qedi: Add ep_state for login completion on un-reachable targets
When the driver finds invalid destination MAC for the first un-reachable
target, and before completes the PATH_REQ operation, set new ep_state to
OFFLDCONN_NONE so that as part of driver ep_poll mechanism, the upper
open-iscsi layer is notified to complete the login process on the first
un-reachable target and thus proceed login to other reachable targets.

2a3d4eb8e2 scsi: flip the default on use_clustering
Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

The QED driver commit, 1ac4329a1c ("qed: Add configuration information
to register dump and debug data"), removes the CRC length validation
causing nvm_get_image failure while loading qedi driver:

where only 00006004 are available
[qedi_get_boot_info:2253]:10: Could not get NVM image. ret = -12

Hence add and adjust the CRC size to iSCSI NVM image to read boot info at

915d9b7142 scsi: qedi: tidy up a size calculation
The id_tbl->table pointer points to unsigned long so static checkers
complain that instead of 4 we should be allocating sizeof(long) bytes.

We're trying to allocate enough bits for the bitmap.  The size variable is
always 1024.  (1024 / 32 * 4) is the same as (1024 / 64 * 8) so this
doesn't change runtime, but this is the more idiomatic way to do it and
makes the static checker happy.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:

as well as handling cases of:

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

1683ce57f5 scsi: qedi: Fix truncation of CHAP name and secret
The data in NVRAM is not guaranteed to be NUL terminated.  Since
snprintf expects byte-stream to accommodate null byte, the CHAP secret
is truncated.  Use sprintf instead of snprintf to fix the truncation of
CHAP name and secret.
