Before this patch, if multiple BO_HANDLES chunks were submitted,
the error -EINVAL would be correctly set but could be overwritten
by the return value from amdgpu_cs_p1_bo_handles(). This patch
ensures that if there are multiple BO_HANDLES, we stop.

Before this commit, only submits with both a BO_HANDLES chunk and a
'bo_list_handle' would be rejected (by amdgpu_cs_parser_bos).
But if UMD sent multiple BO_HANDLES, what would happen is:
* only the last one would be really used
* all the others would leak memory as amdgpu_cs_p1_bo_handles would
overwrite the previous p->bo_list value
This commit rejects submissions with multiple BO_HANDLES chunks to
match the implementation of the parser.

The offset is just 32bits here so this can potentially overflow if
somebody specifies a large value. Instead reduce the size to calculate
the last possible offset.
The error handling path incorrectly drops the reference to the user
fence BO resulting in potential reference count underflow.

The type of size is unsigned int, if size is 0x40000000, there will
be an integer overflow, size will be zero after size *= sizeof(uint32_t),
will cause uninitialized memory to be referenced later.

The basic problem here is that it's not allowed to page fault while
holding the reservation lock.
So it can happen that multiple processes try to validate an userptr
at the same time.
Work around that by putting the HMM range object into the mutex
protected bo list for now.

When p->gang_size equals 0, amdgpu_cs_pass1() will return directly
without freeing chunk_array, which will cause a memory leak issue,
this patch fixes it.

It turned out that not the last IB specified is the gang leader,
but instead the last job allocated.
This is a bit unfortunate and not very intuitive for the CS
interface, so try to fix this.

Allows submitting jobs as gang which needs to run on multiple engines at the
same time.
All members of the gang get the same implicit, explicit and VM dependencies. So
no gang member will start running until everything else is ready.
The last job is considered the gang leader (usually a submission to the GFX
ring) and used for signaling output dependencies.
Each job is remembered individually as user of a buffer object, so there is no
joining of work at the end.

Protect the struct amdgpu_bo_list with a mutex. This is used during command
submission in order to avoid buffer object corruption as recorded in
the link below.

This change adds the dma_resv_usage enum and allows us to specify why a
dma_resv object is queried for its containing fences.
Additional to that a dma_resv_usage_rw() helper function is added to aid
retrieving the fences for a read or write userspace submission.
This is then deployed to the different query functions of the dma_resv
object and all of their users. When the write paratermer was previously
true we now use DMA_RESV_USAGE_WRITE and DMA_RESV_USAGE_READ otherwise.

Instead of tracking the VM updates through the dependencies just use a
sequence counter for page table updates which indicates the need to
flush the TLB.
This reduces the need to flush the TLB drastically.

This is provided by TTM now.
Also switch man->size to bytes instead of pages and fix the double
printing of size and usage in debugfs.
